Mariah’s Story / Pediatric Heart Care Back to Home Page

Baby Mariah was born with a large hole in her tiny heart. She had an atrial septal defect, meaning her heart was missing a section oftissue about the size of a quarter. The only way to repair it was with surgery. The operation was a complete success and Mariah is well on her way to recovery.
Presbyterian is New Mexico’s only hospital that performs pediatric open-heart surgery. Physicians like Dr. Carl Lagerstrom, board certified in pediatric cardiology, represent our dedication to the community.

The Berg Family learns how Presbyterian’s MyChart gives them secure online access to their medical records. MyChart, our electronic medical records tool, makes you an active member of your medical team and helps you keep track of all your medical information. MyChart allows you to view portions of your medical record, including allergy and immunization history. You can receive lab and other test results, e-mail members of your medical team, request appointments, request prescription renewals and pay bills.

MyChart is currently being introduced throughout our system and is expected to be completely rolled out by April of 2012.

Tup’s Story / Complete Cancer Care Back to Home Page

We’ve invested tremendous resources to address New Mexico’s growing need for advanced cancer care. Our new and enhanced services include the Presbyterian MD Anderson Radiation Treatment Center at Kaseman Hospital in Albuquerque. This center provides advanced radiation treatments available nowhere else in the state.

We take a comprehensive approach to cancer treatment. Our team of more than 200 caregivers includes 100 physicians in different specialties. This approach is crucial in understanding and treating cancer, as it affects the entire body and can interact with other diseases and conditions.

No two patients are the same and no two cancers are the same. That’s why Presbyterian provides a wide range of services that include chemotherapy, radiation therapy, hematology, surgery all the way to recovery and rehabilitation. It’s all in the effort to help patients like Tup fight cancer with everything we have.

Donna and Megan’s Stories / A Tour Back to Home Page

Monday, October 24 of 2011 will be a proud day for the Presbyterian family. That’s the day Presbyterian Rust Medical Center Rio Rancho will open its doors. Soon after, Donna and Megan may be two of the first mothers to have their babies here. In fact, their babies could be two of the first born in a Rio Rancho hospital, as we’ll be the first to provide complete obstetric services for the city.

For the first time ever, Rio Rancho and Westside moms will be able to see their doctors for office visits and deliver their babies in the same convenient location close to home. More importantly, they’ll be delivering their babies in New Mexico’s first 21st century hospital with the most advanced care available anywhere.

The new medical center will provide a Level I and II Neonatal Intensive Care Unit (often called a “NICU”) and also includes full gynecological surgical capabilities. So, should the unexpected happen while you’re expecting, there’s no safer place to be.

Alison’s Story / Heart Care Back to Home Page

Four days after giving birth to her fifth child, 31 year-old Alison experienced a massive heart attack. She was fortunate to have been rushed to Presbyterian, with New Mexico’s most advanced cardiac care unit.

Our cardiac program began in 1970 and today we have 21 cardiologists and cardiothoracic-vascular surgeons, including New Mexico’s only pediatric heart surgeon. Our cardiac program includes a Vein and Vascular Center and the state’s largest cardiac
rehabilitation program.

Our physicians, clinicians, and care teams are working every day to transform healthcare. When treating a heart attack, time is precious. Once a patient reaches the doors of the emergency department, the standard, set by the American Heart Association, is to perform angioplasty and open the blocked artery or arteries within 90 minutes. Presbyterian’s average time is 67 minutes, well-exceeding the standard.

Presbyterian Hospital is the only hospital in New Mexico to receive the Gold Performance Award from the American College of Cardiology and American Heart Association for our commitment and success in implementing the highest standard of care for heart attack patients.

Presbyterian recently received the Beacon Award for Cardiac Critical Care Excellence from the American Association of Critical Care Nurses. The award is given to the nation’s top cardiac critical care units and represents an extraordinary commitment to high-quality critical care standards, and recognizes dedication to exceptional care, improved outcomes and greater overall patient satisfaction.

US News & World Report ranked Presbyterian seventh in the nation for re-admission rates in their Best Hospital edition. Our current readmission rate is 19.1% compared to the national average of 24.7%.

Cardiac critical care nurses call patients after they are discharged to make sure their transition from hospital to home is a
complete success.

Peggy’s Story / Hospital at Home » Your Story Is Our Story

Peggy’s Story / Hospital at Home Back to Home Page

Providing the best care possible includes more than advanced technology and highly skilled physicians. Sometimes, it’s as much about where a patient heals as how they’re healed. That’s why Presbyterian’s Hospital at Home program brings the hospital to
the patient.

Patients like Peggy receive the same care they would receive in a hospital in their home. Doctors, nurses and technicians travel to them to provide care. This helps reduce stress, while improving patient satisfaction.

Using new, more portable technology, we’re able to perform x-rays and tests in the patient’s home. Until recently, this level of care was only possible in a hospital setting.

Hospital at Home programs have been shown to offer high patient and caregiver satisfaction, less stress for family members and fewer complications than traditional hospital stays.

Presbyterian partnered with The Johns Hopkins School of Medicine to develop our Hospital at Home program. We’re one of only a handful of hospitals in the nation to offer it.

To date, we have helped more than 300 patients through Hospital at Home. Clinical outcomes are as good, if not better, than patients treated in a hospital. Hospital at Home actually lowers the cost of care by an average of $2,000 per hospitalization. That means we have saved more than $600,000 with the program.

Pastor Hill’s Story / Healthy Solutions Back to Home Page

Some Presbyterian Health Plan members need help understanding their conditions when they are diagnosed with a chronic, often life-changing disease. Education and coaching can improve their ability to better understand their condition and how to manage their health. That’s why we started Healthy Solutions, our Disease Management Program, where trained nurses and coaches reach out to members through scheduled phone calls. This personalized outreach helps members take charge of their condition and care. The nurses and coaches help them reach their health goals for the best possible outcome.

Chronic medical conditions like diabetes and heart disease can be overwhelming at first since many people have to make lifestyle changes such as taking medications and changing their diet. Healthy Solutions creates a partnership, providing support and motivation to members. They are relieved to know they don’t have to deal with their conditions on their own.

Our Healthy Solutions coaches come from a variety of backgrounds, with experience in nursing, social work, counseling and
health coaching.

We have helped more than 1,000 members with specialized coaching through Healthy Solutions. The extra care and coaching doesn’t cost members a penny more.
